Do a compression test. It sounds like you have either a cam off a tooth, causing bad compression, or a bad valve job done, etc. causing your problems. You should never have vacume that low even with huge cams. Check for simple intake vacume leaks as well. It could be something as simple as a crack in your FPR vacume, or intake manifold gasket. So check for all vacume leaks after the throttle body and do a compression test to test the basics.
