Eastwood.com, but if you have the materials and bending skills you could make one yourself pretty easy I'd imagine.
The bushing is used if the hole on your jack top is too large, it helps fill the gap. I have a craftsman (smaller model) and I don't need to use the bushing, it just plops right in.
