The failure I've usually seen is with the spacers that have studs in them, bolted to the hub - in that case, it seems things stretch a bit there and you have to retorque them, taking the wheels off in the process, and ideally retorque the wheels later. This type of spacer could also crack if very poorly designed.
The ones with the studs going through the spacer and into the wheel pretty much require extra-long studs if they're an inch thick, but other than that should be pretty reliable.
