They run them because they want a specific type of fitment or brake clearance they don't make every wheel in everyoffset. they are hubcentric spacers and I would imagine they put just as much stress on the studs/wheel bearings as the same wheel in the lower offset so 17x9.5 +0 = 17x9.5+o wether a spacer is used
