None of the ones listed below are wrong, you don't necessarily need vacuum unless your car has Blow by,then you should have vacuum source. The Positive pressure will be enough to carry vapors into catch can.

There are infinite ways to set up the PCV system. However, the one I'm most interested in seeing is the "trunk monkey" setup. Not many people have trunk monkeys as the vacuum source for thier PCV system but this would be interesting nonetheless. A good catch can is recommended to extend the life of the trunk monkey and reduce costly maintenance. See pic below for proper installation schematic.
