I had my car out this weekend for its once a month drive

and I noticed an odd rattling sound from outside the car. I could not figure out what it was until about 5 miles later my clutch engagement started to get closer to the floor.
By the time I got to the home stretch I had to practically stand on the clutch pedal to shift into gear.
I started checking MC / fluid level and all looks fine
When I went to bleed the slave cylinder I inspected everything down there and noticed that the clutch fork had lots of play in it I could move it around half an inch to an inch up or down and there was some black greasy luid leaking coming out from the dust boot on the fork.
I have searched and found similar problems. . .
I'm kinda stumped here, am I looking at a bad clutch pivot? TO bearing?
I have never heard any grinding or noises from the car until right before I noticed the problem
