ummm....you're not supposed to hear it really....well it's not supposed to sound like that at least, maybe a slight rattle that is bairly imperceptable....sounds like you need a new one or a new guide maybe (those have a tendency to be pieces of crap).....that's my 5 cents

It's a slight ticking noise.  Mine sounded like the fan was hitting some wires.

I just had mine done.  I think the estimate was around $1100.  My extended warranty covered it, so I'm not sure of the exact cost.  If it's only the top guide, it will be much cheaper, since they can access and remove it through the valve cover.  My main problem was the lower guide that's about 7-8" long--the plastic had started to chip away--the new guides are made of metal, instead of plastic.

Get a few estimates from some shops around you that you would trust w/ this kind of job.

Well, you got the fix this noise, otherwise it will getting worse and worse and break down your engine. If you take it to Nissan dealer, they will sure charge you for $1100 for this job. But it is not hard to do it yourself.

The rattling noise is mainly from both TOP timing chain guides.
you don't need to replace them, just open the valve cover and front cover to take them off.

Here is some information about this job and the solution, and this almost included everything you need to know:

Freshalloy timing chain rattling information

and below is the link IF you wish to get the new timing chain kit from Nissan:
