bizarre clutch engagment problem

ok tech heads, ive asked alot of people this and havent gone anywhere with it. i need some help.

problem is when im coming to a stop with my foot on the clutch pedal, my car still wants to die like its in gear. when im at a stop, it wont go into 1st. i have to rev the motor slightly, put it in second then into first. sometimes when im driving it gives me a hard time putting into 4th also. usually while downshifting from 5th.

clutch is an act stage 2, fairly new was put in like 6 months ago or so, by me. properly. pilot bearing, throwout bearing put in correctly. flywheel was resurfaced with no lip, they shaved it off which sucks but i had run one in the past the same way. clutch master was replaced, nismo slave was put on. clutch system was bled.

ive always had clutch problems with this car, and i drive normally, i dont do stupid shit and i dont fuck up the clutch. first clutch i put it was an exedy stage 2, it wore unevenly, then i didnt have money so i put in a used stage 1 exedy and that clutch lasted until death. then i put in another stage 2exedy and the pressure plate went bad or something. so i put in an ACT to try a different brand. i was impressed and liked it better. but then this problem started.

my plans atm are to replace the pivot ball with a nismo, and maybe change the fork, if not i will at least eye it and look for bends or cracks.
