What do you guys think? Turbo fried on a buddies car.....

We took it out a few nights ago and decided to get on it a little it was a stone racing Hybrid T25/T28...The fucker put you in your seat instantly at 1.1 bar.........All of the sudden the thing starts squeelin and makin a weird noise (also spiked to 25 PSI within this period of time but made no boost) On the way to my shop at vaccume it made a loud clanking noise like something went through the piping.....We stayed up all night gettin the mainifold and turbo off (downfall of the SS autochrome mani) Anyway we got it off and low and behold the intake side was chewing into the sidewalls of the turb, the bearing were shot and the shaft was warped.......What the hell caused this? we didnt hit 25 Psi long enough or often enough for that to have been the problem.....>

Only thing I can think of is when the wastegate (flapper) opened it was oblong and got stuck on the 02 housing (stuck open) with a free spooling turbo at less than boost level RPM's and it just spun to fast...............
