Tranny Diagnosis..Help Please

I know my TOB is bad because I can hear it knocking at idle and when I press the clutch in it stops. Another thing it does is at first it only did it in reverse. I would notice lots of noise/vibration from shifter first then when slipping the clutch to get moving there is this very un-settling whirling/grinding (idk) its weird. It used to do it once a week or so and now its daily in 1st, 2nd, and reverse. I've also been grinding a lot of gears lately too. I even go as far as taking a second or 2 between shifts and making absolutley sure the clutch is fully pressed in; 3rd and 5th are the worst. If I put 3rd to 5th at like 3500-4k it grinds every time. Is the tranny trash or what? Thanks in advance
