a regular 12mm socket, 2 6" extentsions, a wobble joint or u-joint and one of those retractable grabbers. It's a lot easier if you don't have the egr hooked up or the extra brackets on the bottom of the mani. With that stuff on there it can be a real pain in the ass, but without it, it's cake besides the heater core lines. But if you ever do have the mani off, i would recommend installing the water temp sensor in the manifold.
