Urban legend. People think of the old painted headers that flake off. Ceramic coating is like Powder Coating. It does not flake off.

The inside should be coated to keep heat from transferring into the metal and slowing the exhaust charge. The outer coating is for appearance, environmental protection and further heat protection.
