Facking pressure plate

My car has been making this squeaking noise the past couple of weeks. And it goes away when I press the clutch. Never really thought to much about it.

So I'm about to turn on a highway. Pulled out of 5th into neutral, slowed down, tried to put the car in 3rd, and it wouldn't budge.

Wtf? So i pull over, check my fluid level, slave cylinder, blah blah blah, its all fine doing it's job.

Get back in the car, start it up in gear and the car kind of jumps forward. wont stall out but you can hear the clutch and flywheel rubbing and screeching.

It shifts fine on the way home. I let it sit for 15 mins or so, and go mess with it again. Now it wont go into gear unless the car is off.

So I pull the tranny the next day, and I fine this.

Wtf? looks like someone took a torch to it, the teeth sit basically flat now and there are big as grooves in them as well.

So i found my problem, but im wondering what caused this? This is a spec stage 1 w/only 3k miles, Bought it brand new last October.

Heres another
